A participant survey that produces usable answers, sent, collected, and read for you. Describe your program and what you need to learn, and Taskade Genesis writes the questions, sets the scales, builds the collection sheet, and summarizes the responses into themes with the quotes that support each one.
The app embedded on this page is Feedback Survey, a real Taskade Genesis build that collects responses and summarizes them as they land. Read the question set, add a response, watch the summary shift, then clone it in about ten seconds and rewrite the questions into the program evaluation survey described above.
What you get is a full evaluation instrument plus the analysis. The question set covers reach, satisfaction, perceived change, barriers encountered, and open feedback, written at a reading level your participants can handle and short enough that people finish it. Each response row holds the answers, the program, the date, the collection method, and any follow-up flag. Linked records connect responses to the indicators in your outcome tracker so the survey feeds your reporting rather than sitting in a separate file.
Workspace DNA turns responses into decisions. Memory keeps prior waves, so this survey is comparable to the last one rather than a standalone snapshot. Intelligence, backed by 15+ frontier models from OpenAI, Anthropic, Google, and open-weight providers, codes open-text answers into themes, counts them, pulls the representative quote for each, and separates a real signal from three loud responses. Execution flags anyone who asked for follow-up and creates the task for a staff member.
Read the results in whichever layout you need:
- Table view, every response with scores and themes in a grid
- List view, the coded themes with supporting quotes for a report
- Board view, open feedback grouped by theme and priority
- Calendar view, survey waves and collection windows across the year
- Plus Mind Map, Gantt, and Org Chart across the 7 project views
Automations run the fieldwork. With Taskade automations, a completed service episode triggers the survey invitation, a non-response sends one polite reminder, and a low satisfaction score creates an immediate follow-up task rather than waiting for the analysis. Across 100+ bidirectional integrations, your form tool and Google Sheets pull responses in while Gmail and Slack push invitations and alerts out.
The AI agents carry 34 built-in tools including persistent memory, so an agent can read 200 open-text answers, return six themes with counts, and tell you which one changed most since the last wave.
Clone it, invite your program and evaluation staff, and the instrument, the responses, and the analysis stay in your workspace.

Frequently Asked Questions
How long should the survey be?
Short enough to finish. The app defaults to roughly eight to twelve questions with one or two open fields, and it will warn you when an addition pushes the completion time past the point where response rates fall.
Can it write questions that avoid leading the respondent?
Yes. The agent flags leading phrasing, double-barrelled questions, and scales without a neutral option, and proposes neutral alternatives. You approve the final wording.
Can we run it in more than one language?
Yes. Taskade Genesis routes to 15+ frontier models including multilingual-capable ones, so the instrument can be translated and responses coded across languages in one dataset.
What about participants without email?
Collect on paper or by phone and enter the responses, or hand a tablet to a staff member at the site. The analysis works the same regardless of how the answers arrived.
Does it protect anonymous responses?
If you mark a wave anonymous, identifying fields are not collected and the follow-up flag is disabled for that wave. Choose that setting when candor matters more than the ability to follow up.
Can we compare this year to last year?
Yes. Keep the core questions stable and each wave becomes comparable, with the app showing movement per question rather than making you line up two spreadsheets.
Does this replace a formal evaluation?
For most small organizations it is the practical version of one. If a funder requires an external evaluator, this is the data collection that makes their job faster and cheaper.
