🚨 Handle Incidents When AI Gets It Wrong

Put a sensible process around how your organisation uses AI, without stopping anyone getting work done. These prompts build the approval queue, the register of what is running, the log of what each tool actually did, and the policy an assistant checks submissions against. Written for the person who has been asked to own this and given no software to do it with.

When an AI output goes wrong we fix it and move on without recording anything. Build me an incident register. Record each incident with the date it occurred and the date it was identified, the use case and tool involved, what happened in plain language, who or what was affected, the severity, and crucially how it was detected, since being told by a customer is a very different finding from catching it internally. Include near misses where nothing actually went wrong. Record the immediate action taken, the root cause, and corrective actions with owners and due dates, tracked through to completion rather than to agreement. Link each incident to the use case and the policy rules involved. Add an AI agent that reviews incidents over time and tells me plainly where the same underlying cause keeps recurring, which controls are not working as intended, and which detection gaps mean we are probably missing incidents entirely. Automate three things: an alert to the governance owner on any high severity incident, reminders on overdue corrective actions, and a scheduled quarterly themes report. Three logins: anyone can report an incident, an owner works corrective actions, and an auditor reads everything.

Handle Incidents When AI Gets It Wrong

Record AI incidents and near misses with impact, detection method, and corrective actions tracked to completion.

// the spark catches · intelligence awakens · the answer unfolds

AI will produce a wrong or inappropriate output eventually. What matters is whether you noticed, what you did, and whether anything changed. This records all three.

What's Included

  • Every incident recorded however small it seemed.
  • Impact and detection captured because how you found out matters.
  • Actions taken and completed not just described.
  • An assistant that finds the pattern across incidents.

How To Use

  1. Record near misses too. They are the cheapest lessons available.
  2. Capture how it was detected. If a customer found it, that is the finding.
  3. Track corrective actions to completion, not to agreement.
  4. Review themes quarterly. Repeats mean the control did not work.

Link incidents to their use case, and feed changes back into the policy register.
